The skipper is back alongside Raphael Varane in the heart of defence, with Nacho dropping to the bench, as Los Blancos look to build on the 4-1 victory at top-four rivals Valencia last weekend.
Cristiano Ronaldo, who scored two from the spot against Los Che, is up front alongside Gareth Bale and Karim Benzema, while Luka Modric, Casemiro and Toni Kroos keep their places in midfield despite the return from injury of Isco, who is on the bench.
Keylor Navas makes a return to the club he represented between 2011 and 2014 and is in between the sticks, while Dani Carvajal and Marcelo line up either side of Ramos and Varane.
Options on the bench for Zinedine Zidane, whose side are 19 points behind leaders Barcelona, include Lucas Vazquez, Marco Asensio and Mateo Kovacic.
As for the hosts, who sit 16th in the table and two points above the drop, Juan Muniz makes four changes following the 2-2 draw at Deportivo La Coruna a week ago.
Muniz is expected to play a 4-4-2 formation, with Jason dropping to the bench to accommodate the inclusion of Ghanaian striker Emmanuel Boateng alongside Ivi up front.
Jose Campana is suspended for the clash, meaning Torino loanee Sasa Lukic gets a start in the midfield alongside Jefferson Lerma, Cheick Doukoure and Jose Luis Morales.
Rober Pier returns to the lineup after being suspended against his parent club last week and Sergio Postigo earns a recall in the heart of defence, with Chema suspended and Erick Cabaco among the substitutes. Coke is at right-back, Antonio Luna is on the left and Oier Olazabal is between the posts.
