Exeter took control of the match early on, tries from Ian Whitten and Simon Alcott helping the home side to a 17-3 lead inside the opening 20 minutes.
However, a yellow card for James Hanks allowed Scarlets to fight their way back into the match, going in to half time just seven points behind.
A Scott Williams try at the start of the second half levelled the scores, forcing Exeter to make a series of changes as they sought to regain their early dominance.
The home side re-took the lead through Gareth Steenson, who had missed two penalties during the match, before James Scaysbrook sealed the win in the final moments of the game.
The win means that the Chiefs have two wins from four games in the Heineken Cup, the same record as second-placed Leinster.
