Hello everyone! It's turning out to be another wonderful season in the Premier League and there are more points to play for on a busy afternoon. There are five games kicking off at 3PM, and here our focus will be on the events at Upton Park, as high-flying West Ham United take on an out-of-sorts Chelsea.

West Ham have enjoyed a wonderful start to the season and they are in the top four after winning five of their opening nine matches. However, Slaven Bilic's side have won just one of those games at home, and their supporters will be desperate to see their team improve in their final season at Upton Park.

Meanwhile, Chelsea's struggles over the past couple of months have been widely documented and they currently sit eight places below the Hammers. They did mange to beat Aston Villa last weekend, but a trip to Dinamo Kiev in the Champions League could provide a nasty hangover this afternoon.

With Chelsea in European action over the past week Jose Mourinho may have been tempted to make some changes to his side. So, without further ado, let's have a look at which players are in the starting lineups for today's clash.

WEST HAM XI: Adrian, Jenkinson, Tomkins, Collins, Cresswell, Noble, Kouyate, Zarate, Payet, Lanzini, Sakho

West Ham have been forced into making one change to the side that beat Crystal Palace last weekend. Victor Moses is unable to play against his parent club, so Argentine Mauro Zarate is brought into the starting lineup.

As for Chelsea, Jose Mourinho has decided against changing his team around following the long trip to the Ukraine, and has named the same side that started in the goalless draw at Kiev. It does mean that there is a start for Eden Hazard, who was dropped to the bench last weekend, while Kurt Zouma continues at right back in place of the injured Branislav Ivanovic

Well, I'm sure that many West Ham fans could not have imagined the great start they would have had to the Premier League season, after suffering an early exit in the Europe League. They responded to that disappointment in perfect fashion with a victory over Arsenal on the opening day, and since then they haven't looked back.

Stunning victories away from home over Liverpool and Manchester City followed, and they won their fourth match on the road last weekend against Crystal Palace. That match had looked like ending in a draw, but late goals from Manuel Lanzini and Dimitri Payet sealed a 3-1 victory at Selhurst Park.

Despite their success outside of East London, they have only won once on home turf, which was a 2-0 victory over Newcastle United in mid-September. They have lost two games at Upton Park, to Leicester City and Bournemouth, while they were forced to fight back to earn a 2-2 draw against Norwich City in their previous home outing last month.

The style of counter-attacking football that they are playing under Slaven Bilic is certainly custom made for their away fixtures, which perhaps explains their problems at home. No matter where they play they have been freely finding the back of the net, with their tally of 20 goals only surpassed by league leaders Manchester City.

One of their summer signings has played key part in their fine attacking play, as Dimitri Payet has scored five and set up three of his team's league goals this season. The Frenchman has impressed hugely since arriving from Marseille and is already one of the frontrunners to win the PFA Player of the Year award.

Usually whenever these teams met, it is Chelsea who are ahead of West Ham in the table, but a poor start to the season for Jose Mourinho's side has left them in 12th spot. The defending champions have struggled to find any form or consistency and they have already lost four of their nine league games.

Even when they have been winning the Blues have not looked back to their best, and they did little to silence their detractors last weekend against Aston Villa. Diego Costa's first-half goal gave them the lead, and after the break the Brazilian saw an effort deflect off Alan Hutton to earn the Blues a much-needed 2-0 victory.

They returned to action three days later in Ukraine, with a clash against Dynamo Kiev in the Champions League. Having lost against Porto in their previous group match, a positive result was of vital importance, but they could only manage a goalless draw, despite hitting the woodwork twice and having a good shout for a penalty turned down.

This is a fixture that the Blues have dominated in recent years, and they have lost just one of the past 19 games against the Hammers. On that occasion in December 2012, goals from Carlton Cole, Mohamed Diame and Modibo Maiga earned West Ham a 3-1 win, after Juan Mata had given Chelsea the lead.

Since then, Chelsea have won both of their matches at Upton Park, with a strike from Eden Hazard sealing a 1-0 victory in March. They had previously recorded a 2-0 victory over the Hammers at Stamford Bridge on Boxing Day, with John Terry and Diego Costa on the scoresheet.

The two teams have just made their way out onto the pitch and to a rapturous applause from the supporters. We're now just three minutes away from the final Premier League match between West Ham and Chelsea at Upton Park.

KICKOFF! We are underway at the Upton Park!

You can already tell that this is a derby match, as there have been plenty of physical conformations between the teams. There haven't been any chances just yet, but it appears that we are set up for a fascinating clash this afternoon.

SHOT! Payet has drifted into some promising areas so far and he breaks away from his marker to receive the ball in space just inside the Chelsea half. The Frenchman advances and is not closed down, so decides to try his luck from range, but he ends up firing the ball well over the bar.

It has been a very even start to the match, with each team enjoying spells in possession. The Hammers have looked more dangerous going forward, but their latest break down the left ends when Payet is flagged offside.

CHANCE! We've seen the first big opening of the match and it comes to Chelsea. Fabregas flicks a delightful pass over the top of the defence for Costa, who brushes off his marker to get to the ball. He then takes a touch and looks to smash an effort past Adrian, but instead he gets underneath the ball and puts it high into the stands.

I mentioned how dangerous West Ham have been on the counter attack this season, but in the early stages Chelsea have also shown their threat. Willian leads their latest charge forward with a powerful burst down the right, before his final ball into the middle is well cut out by the Hammers defence.

SHOT! Costa is starting to look a real threat for Chelsea and he does well to win a corner down the right after some good hold up play. When the delivery is whipped into the box, it's Costa who rises to meet it, but he can only direct the ball wide of the far post.

SAVE! Payet fired over the bar from long range earlier in the half, but he gets the chance to shoot from closer when he wins a free kick around 25 yards out from goal. He then fires in a powerful effort, but Begovic is there to touch the ball over for a corner.

From the resulting corner, West Ham take the lead thanks to a brilliant finish from Zarate. Chelsea struggle to clear the set piece which lands at the feet of the Argentine, who then takes a touch before crashing a fine low strike past the reach Begovic.

Chelsea look for a quick response and they break quickly with Hazard leading the charge down the left. The Belgian then plays the ball into Costa inside the box, but the Brazilian's touch lets him down and West Ham scramble it clear.

YELLOW CARD! Azpilicueta picks up the first booking of the match for a foul on Zarate by the touchline.

SHOT! This is a lot better from Chelsea, as they work the ball neatly to Ramires, who is in space just outside the box. The Brazilian fancies his chances and lashes in a strike, but it goes well over the bar and fails to trouble Adrian.

At the moment, West Ham are sitting back and are allowing Chelsea to have plenty of time on the ball. They are just waiting for their chance to steal possession and hit their opponents on the counter, and it is a tactic that has worked well for them this season.

Jenkninson puts the Hammers in all sorts of trouble when his misplaced header back to Adrian ends up going out for a corner. Fabregas then takes the resulting set piece, but the West Ham keeper bails his teammate out of danger by coming for the ball and getting a strong hand to punch it clear.

SAVE! The visitors are given a free kick just outside the box when Ramires is brought down by Cresswell. Willian steps up to take the set piece and tries to curl a low effort into the corner, but it is read well by Adrian, who quickly springs across his line to palm the ball to safety.

OFF THE LINE! Chelsea come agonizingly close to getting back on level terms as Zouma rises highest to meet a cross from Fabregas and heads goalwards. Lanzini then gets a touch on the ball in front of Adrian and diverts it towards his own net, before he gets back quickly to clear the ball off the line.

YELLOW CARD! The Blues are fortunate not to go down to 10 men as Matic brings down Kouyate, with the West Ham player driving through on goal. As there was a covering defender he is only shown a yellow card, but the threat of the counter attack was evident once again by the Hammers.

CHANCE! The main chances for both teams have come from set pieces this afternoon and Payet's latest free kick into the box is flicked on by Collins. The defender gets plenty of power on his header, but Begovic is well placed to make a save and hold onto the ball.

CHANCE! How big a miss will this prove to be for West Ham? Lanzini finds himself through on goal after being played in by a brilliant flick from Payet. He is faced with just the keeper to beat, but he goes for the chip and puts the ball a few inches over the bar. What a great opportunity that was for the Hammers just minutes before the break.

GOAL DISALLOWED! The attacking play from both teams on the break has been fantastic to watch this afternoon and Willian shows his pace to drive into the West Ham half following a corner. He then gets to the edge of the box and plays it out wide to Fabregas, who fires a low strike into the far corner, but the goal is ruled out for an offside against the Spaniard. It was an incredibly tight decision and Cresswell may have been playing him on, but West Ham have got off lightly.

RED CARD TO CHELSEA! Things go from bad to worse for Chelsea, as Matic is shown his second yellow card of the match. The Serbian had dragged down Sakho, as the forward went past him down the right, and there can be no complaints about the decision.

YELLOW CARD! The Blues have lost their cool and they surround the referee to argue over the decision to send Matic off. Fabregas and Costa were most vehement in their complaints and they both pick up bookings for their protests.

There are going to be at least a couple of minutes added on to the end of the first half.

What an explosive half of football that was at Upton Park! It has been a truly engrossing watch and it is West Ham who are in the lead. Mauro Zarate's strike is the difference, but Chelsea will have to play the second 45 minutes with 10 men, following Nemanja Matic's red card.

There have been so many talking points and most of the big decisions came at the very end of the half. Firstly, Cesc Fabregas thought that he had levelled the scores, but his effort was ruled offside. From that moment, the Blues seemed to lose their heads, and Nemanja Matic was dismissed for a poor challenge. He was perhaps fortunate not to be sent off earlier, but he has now put his team in real danger of losing their fifth league game of the season.

In terms of the statistics, it is Chelsea who have bossed possession and the visitors have seen 63% of the ball. However, West Ham have had six attempts on goal, compared to five from Chelsea, and they have looked so dangerous on the break all afternoon.

RESTART! West Ham take the kick off and get the second half started. There has also been a change at the break made by Chelsea with Mikel coming on to replace Fabregas.

We've just seen a shot of Mourinho, who is sitting up in the stands at the start of this half. It's unclear at the moment whether the referee has sent him off and banned him from the touchline, so we will have to wait and see whether he stays there for the remainder of the match.

It will be interesting to see if West Ham try to press forward and take a more commanding hold of the match. Many of their best moments have come on the break, but they are bound to see a lot more of the ball now, given their man advantage.

Wonderful piece of play from Cresswell down the left, who works some space and fires a teasing delivery into the box. The ball fizzes right across the six-yard box and Sakho makes a late run to meet it, but the striker can't quite stretch enough as it goes harmlessly out of play.

Would you believe it! Chelsea are back on level terms and once again the goal has come from a corer. This time it's poorly defended by the hosts, and they fail to win it in the air, with the loose ball dropping kindly for Cahill, who turns sharply and smashes a low drive past Adrian. Mourinho is shown looking stone-faced in the crowd, but I'm sure he will be delighted at the response from his team.

CHANCE! Payet has been quiet since the break, but he shows his class by driving down the left, before cutting back onto his right right. He then clips the ball to Noble, who knocks down a header for Zarate to fire a low half-volley straight into the gloves of the keeper.

It's not quite happening for West Ham at the moment, as Cresswell delivers a cross from the left that is far too high for any of his teammates inside the box. The Hammers need to take a bit more care whenever they get into the final third, as they do have Chelsea pegged back for large spells.

If we are going to see a third goal this afternoon, then I would guess that it will come from another set piece. Neither team have covered themselves in glory and have defended poorly, and on this occasion Costa gets above everyone to head over a Willian free kick, although the flag was raised against the striker.

SUB! West Ham are going to make their first change of the match, with goalscorer Zarate making way for Carroll.

Outrageous skill from Payet, who is picked out down the left by a long ball from Noble, before taking a touch and flicking the ball over the head of Willian. He then fires the ball into the box on the volley, but it lacks direction and goes harmlessly wide to the right.

Chelsea are enjoying their best period of the half, and have been able to work the ball around neatly between their midfielders. They still have plenty of players capable of scoring a winner late on, and even with 10 men they have managed to get on the scoresheet.

YELLOW CARD! Kouyate is the first West Ham player to be booked this afternoon for a poor challenge through the back of Ramires.

Begovic is almost caught out as he receives a pass back from Terry, before taking a heavy touch. Sakho is alert and is quickly in to closer the keeper down, but he manages to recover in time to put the ball out for a corner.

CHANCE! Big, big moment in the game for Chelsea, as they clear the corner from West Ham, before charging forward on the break. Hazard then tricks his way down the left and pulls the ball across the box for Ramires, but the Brazilian scuffs his effort and Adrian is able to make a simple save.

This is exactly why Carroll was brought on for the Hammers! The England international shows his quality in the air by rising above everyone in the box to meet Cresswell's cross, before guiding a thumping header beyond the keeper and into the far corner. Will the hosts be able to hold onto their lead or can Chelsea fight back once again?

SHOT! What a sensational goal this would have been had it gone in, as Willian tries his luck from the kick off. Adrian was slightly off his line and looked worried for a second as he rushed back, but the shot ends up drifting a few yards over the bar.

YELLOW CARD Willian is the latest Chelsea player to be booked for a foul on Carroll.

SUB! It's a change for the Hammers, as Obiang is on to replace Lanzini.

SUB! Chelsea also make a substitution, with Falcao brought on to replace Ramires.

The Blues are starting to pile on the pressure, but the hosts are able to deal with everything that is coming their way. There is also plenty of space for the Hammers to attack down the flanks and Kouyate does well to break at pace before winning his team a corner.

SUB! Third and final change of the match for Chelsea, as Rahman comes on for Azpilicueta.

There are going to be at least four minutes of stoppage time.

This could be the moment for Chelsea to get back into the match, as they win a free kick just outside the box following Sakho's foul on Hazard.

SUB! Ogbonna is coming on for the Hammers to help them defend the free kick, with Noble the man to make way.

Willian finally takes the set piece, but it is not his best delivery, as his effort is easily headed clear by the Hammers. Hazard then picks up the loose ball, but he is unable to fashion a chance as the Hammers survive.

YELLOW CARD! Mikel is the sixth Chelsea player to be shown a card for a rash tackle on Payet. That is surely the last action of the match, with the referee set to call time.

Incredible scenes at Upton Park, as West Ham hold on to claim a 2-1 win over Chelsea and move up to second in the Premier League. In addition to beating the Blues, the Hammers have earned victories over Arsenal, Liverpool and Manchester City this season, as their remarkable form shows no signs of stopping.
