The visitors thought that they had opened the scoring less than a minute after kickoff when the ball ended up in the back of the net following a corner, but the ball was judged to have gone out before being tapped in by Leroy Fer, so the referee chalked it off.
QPR continued to look lively early on, with Fer dodging a challenge from Steven Gerrard before rifling the ball towards goal, but Simon Mignolet made the stop.
Liverpool's first chance of the afternoon fell to Gerrard after Joey Barton gave the ball away in midfield, but the captain's finish was wide, while Rickie Lambert was denied by Robert Green a couple of minutes later.
The home side broke the deadlock shortly after as Lambert cut inside before finding an unmarked Coutinho, setting the forward up to curl the ball into the top corner.
Liverpool also had a goal ruled out for offside in the first half when Raheem Sterling fired home after a flick-on from Henderson.
After Charlie Austin tried to level the scores with a curling effort from 20 yards out, Green was tested again before the break as Gerrard got forward, but the keeper tipped it behind with his fingertips.
